Chinook began using Velvac mirrors in mid '95.

I made a thread about adjusting/tightening the Velvac 2015 model mirrors, and it includes some photos that show how to identify them. Here is a link to that thread:

The black trim around the head doesn't look familiar (added Trim Lok by PO?), but the arm and part on the van look like it could possibly be a Velvac 2015. Perhaps you will be able to tell if you take a look at the above thread, since you have the mirrors on hand to look at.
I don't know when Chinook started using the Velvac 2015 but they seemed to stop in around 1999. After that they went to the Velvac 2020.
If you do have the 2015 they are out of production but some parts can still be purchased. The 2020 is still a current model (but it's expensive to switch if you don't have to). Mirror glass is one of the parts that can be purchased for the 2015. Here is a link to one site (friend did purchase from them and had good experience):

Looks identical to mine, when I recall by memory. Will take a look when I get home, to confirm.
[edit] Just came back from outside. The mechanism that attach the arm to the door is same, but the mirrors are different. Mine has no black trim.

Yep, my mirrors are an odd ball, they don't match anything out there. You have to wonder what Chinook was doing while these classics were on the production floor.

I did find a company that makes generic mirror glass. It's all good.....

Glad to hear you found something workable.
There were always little things changing from year to year, but it does seem like the early-to-mid 90's had a lot more variation. Things seemed to have settled down around 1997 when the new engine family came in (Ford). Might be just a coincidence, but anyway, after that there were just little changes in the manual from year to year, and each year seemed pretty consistent within itself.

Well, had no luck at all finding replacement mirror glass. Went to a local auto glass store and they quoted $125.00 for the mirror glass. So, went down to a auto parts store and found a truck mirror, took it apart and glued the mirror glass over my mirror with the broken edge in the picture. The edges of the new mirror touch the rubber gasket, so works fine. $36.00.
